The USCIS Report of Medical Examination and Vaccination Review Report (Form I-693) is required for all persons seeking permanent residence in the United States. This includes green card applicants, visa applicants and, refugees and asylum seekers.

All parts of your medical examination are performed by the civil surgeon. This includes a medical history review, immunization records review, ordering and reviewing your screening laboratory tests, a physical examination and completion of your I-693.

You will also have access to the civil surgeon during the process of your immigration medical exam to answer any questions regarding exam requirements.
